Practicum-Evaluation-Site-Supervisor<\/a>\u00a0This form is to be completed by the Site Supervisor at the end of the semester to evaluate the practicum student.<\/span><\/h3>\n

Intern-Evaluation-By-Site-Supervisor<\/a>This form is to be completed by the Site Supervisor at the end of the semester to evaluate the internship student.<\/span><\/h3>\n

Log Sheet<\/a>\u00a0This is the log sheet that both practicum and internship students use to log hours at their sites.<\/h3>\n

College Of Education Graduate Programs- Disposition Expectations And Indicators \u00a0<\/a>This form outlines the four COE Dispositions applicable to graduate students.<\/h3>\n

Dispositions are of such importance that problems with attitudes and specific behaviors often can and do impact or even negate a student\u2019s knowledge and skills.\u00a0 Thus, the instructor (and site-supervisor) during any field experience reserves the right to substantially alter a student\u2019s grade if, in his\/her professional judgment, the dispositional concerns are of a magnitude to warrant this action. \u00a0The COE dispositions are as follows:<\/h3>\n

    Displays appropriate level of professional behavior<\/h3>\n<\/li>\n
  2. \n

    Demonstrates the belief that all students can learn.<\/h3>\n<\/li>\n
  3. \n

    Shows appreciation and skills needed for teaching, learning, and mentoring.<\/h3>\n<\/li>\n
  4. \n

    Demonstrates collegiality and collaboration.<\/h3>\n<\/li>\n<\/ol>\n

    *If for any reason, you, as the site supervisor have any<\/strong> concerns regarding \u00a0student dispositions, please speak with the student directly and\/or contact the student’s faculty supervisor as soon as possible.<\/em><\/h3>\n
