Click the <img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-253 size-full\" src=\"http:\/\/blogs.butler.edu\/mybutlerhelp\/files\/2012\/09\/save_green.gif\" alt=\"save button\" width=\"47\" height=\"27\" \/>button.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<h4><a id=\"phone\"><\/a>To Change Your Telephone Numbers<\/h4>\n<ol>\n<li>\u00a0In the <strong>Student Center<\/strong>, under the <strong>Personal Information<\/strong> heading, click on the <strong>Home Telephone<\/strong> link.<\/li>\n<li>Edit your phone numbers as desired. To add a phone number, click the <strong>Add a Phone Number <\/strong>button.<\/li>\n<li>When finished, click the <strong>Save<\/strong> button.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<h4><a id=\"email\"><\/a>To Add an Email Address<\/h4>\n<ol>\n<li>In the <strong>Student Center<\/strong>, under the <strong>Personal Information<\/strong> heading, select <strong>Email Addresses<\/strong> from the dropdown and click the <img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-119 size-full\" src=\"http:\/\/blogs.butler.edu\/mybutlerhelp\/files\/2012\/09\/double_arrow.gif\" alt=\"go button with double arrow\" width=\"19\" height=\"19\" \/>\u00a0button.<\/li>\n<li>Click the <strong>Add an Email Address<\/strong> button.<\/li>\n<li>Select the email type of &#8220;Other&#8221; and type in your email address.<\/li>\n<li>Click the <strong>Save<\/strong> button.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<h4><a id=\"contact\"><\/a>To Add an Emergency Contact<\/h4>\n<ol>\n<li>In the <strong>Student Center<\/strong>, under the <strong>Personal Information<\/strong> heading, click on the <strong>Emergency Contacts<\/strong> link.<\/li>\n<li>If you have never supplied an emergency contact before, click the <strong>Add an Emergency Contact<\/strong> button.<\/li>\n<li>Complete the address and phone options following the onscreen instructions.<\/li>\n<li>When finished, click the<strong> Save<\/strong> button.<\/li>\n<li>Click <strong>OK<\/strong> on the confirmation screen to return to the <strong>Emergency Contacts<\/strong> page.<\/li>\n<li>If you have more than one emergency contact, designate the primary contact by clicking the <strong>Change the primary contact<\/strong> button and selecting your desired primary contact from the dropdown list. Click <strong>Save<\/strong> and then <strong>OK<\/strong> on the confirmation screen to return to the <strong>Emergency Contacts<\/strong> page.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<h4><a id=\"ethnic\"><\/a>To Indicate Ethnicity<\/h4>\n<ol>\n<li>In the <strong>Student Center<\/strong>, under <strong>Personal Information<\/strong>, click the <em>Demographic Data<\/em> link.<\/li>\n<li>Click on the <strong>Ethnicity<\/strong> tab.<\/li>\n<li>Two questions are presented. Click the <em>Explain<\/em> link for an explanation of each, if needed.<\/li>\n<li>Select the appropriate radio button to answer the first question, and click the checkbox next to one or more choices on the second question.<\/li>\n<li>Click to check the checkbox next to <em>The Information is correct as entered<\/em>, then click the <img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-279 size-full\" src=\"http:\/\/blogs.butler.edu\/mybutlerhelp\/files\/2012\/09\/submit_green.gif\" alt=\"submit button\" width=\"172\" height=\"24\" \/>\u00a0 button.<\/li>\n<li>If you want to optionally further identify your ethnicity, click on the <img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-37 size-full\" src=\"http:\/\/blogs.butler.edu\/mybutlerhelp\/files\/2012\/09\/add_ethnicity_info.gif\" alt=\"Add ethnicity information button\" width=\"175\" height=\"24\" \/> button.<br \/>\n<img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-144 size-full\" src=\"http:\/\/blogs.butler.edu\/mybutlerhelp\/files\/2012\/09\/ethnicity_2part.gif\" alt=\"Multi-ethnic selection options\" width=\"681\" height=\"486\" srcset=\"http:\/\/blogs.butler.edu\/mybutlerhelp\/files\/2012\/09\/ethnicity_2part.gif 681w, http:\/\/blogs.butler.edu\/mybutlerhelp\/files\/2012\/09\/ethnicity_2part-300x214.gif 300w\" sizes=\"auto, (max-width: 681px) 100vw, 681px\" \/><\/li>\n<li>Select the desired descriptor from the <em>Background<\/em> dropdown. Click the <em>Return to Student Center<\/em> link to exit.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<h4><a id=\"name\"><\/a>To Update Your Name<\/h4>\n<p>To update your <strong><em>Preferred Name<\/em> <\/strong>(which is reflected on the class roster and in Moodle),<\/p>\n<ol>\n<li>In the <strong>Student Center<\/strong>, under the <strong>Personal Information<\/strong> heading, select <strong>Names<\/strong>.<\/li>\n<li>Click the <strong>Edit<\/strong> button and type in your preferred first, middle, and last name.<\/li>\n<li>If the new name is to take effect at some point in the future, please insert that date into the <strong>Date Changes Will Take Effect<\/strong> field.<\/li>\n<li>Click the <strong>Save<\/strong> button.<\/li>\n<\/ol>\n<p>To update your <strong><em>Primary\/Legal Name<\/em><\/strong> (which is reflected on your official transcript), please complete the <a href=\"https:\/\/www.butler.edu\/registrar\/name-gender-address\">Name Change Form<\/a> (PDF) and submit it, along with a copy of your <a href=\"https:\/\/faq.ssa.gov\/ics\/support\/kbanswer.asp?QuestionID=3749\">Social Security<\/a> card reflecting the new name, to:<\/p>\n<ul>\n<li>Registration and Records<br \/>\nJordan Hall 133<br \/>\n4600 Sunset Avenue<br \/>\nIndianapolis, IN 46208<br \/>\nFAX: 317-940-6539<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h4><a id=\"orientation\"><\/a>To Indicate Sexual Orientation<\/h4>\n<ol>\n<li>In the <strong>Student Center<\/strong>, under <strong>Personal Information<\/strong>, click on the <strong>Sexual Orientation<\/strong> link.<\/li>\n<li>Click to select the desired radio button next to your orientation.<\/li>\n<li>Click the\u00a0 <img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-276 size-full\" src=\"http:\/\/blogs.butler.edu\/mybutlerhelp\/files\/2012\/09\/submit_and_save.gif\" alt=\"Submit and save button\" width=\"110\" height=\"25\" \/>button.
