Medical Social Worker

A Medical Social Worker assists patients and their families with problems related to home care and coordination of community resources. Their focus may include:

• Supporting clients in their home health service needs
• Offering support and guidance for rehabilitation
• Providing coaching about life and its possibilities
• Providing counseling services for bereavement and grief
• Evaluate clients psychosocial status
• Provide appropriate community resources

Medical Social Workers coordinate with various organizations in their pursuit to assist clients in finding resources and support to foster their healing. They are highly familiar with what the community can offer and are very adept in assessing clients’ psychosocial functioning as well.
