Introduction

With an increase in patients with Type II Diabetes visiting the Diabetes & Endocrinology Center due to Franciscan Health ACO requirements, our service will allow the physicians and specialists to continue with their normal work flow and focus on the patients they currently see without worrying about an influx of patients in the same amount of time. The pharmacist is qualified to speak with patients, educate on their disease state and their medications, and recommend therapy regimen changes through their Doctorate of Pharmacy degree, Ambulatory Care Board Certification, and extended education through a PGY1 and PGY2 residency. A pharmacist is the best healthcare professional for the job because diabetes management involves lifestyle changes AND optimal medication regimens, both of which the pharmacist is knowledgeable of and excels in. A pharmacist has the ability to put the time and effort into patients struggling with their disease that other healthcare professionals may not have.
