The addition of a full time clinical pharmacists to the geriatrics clinic at Community Health Network will allow for patients to get individualized care when it comes to their medications. Community prides themselves on putting the patient first, and this service will do just that. We will put our patients first by assuring them the safest, most effective drug therapy regimen with complete education. By adding a medication expert, Community Health Network will see a decrease in adverse drug events caused by polypharmacy creating a cost avoidance, as well as direct revenue coming in from annual wellness visits.
