Marketing Plan

Product:

  • Our clinic service offers a comprehensive geriatric assessment including but not limited to psychosocial evaluation, medication review, medical interventions and lab work as needed.
  • Pharmacist specifically focuses on medication review, optimization of therapy and Medicare annual wellness visits.
  • We work with both the patients and caregivers to meet any needs in order to maintain an optimal level of health

Price:

  • Pharmacy services will not be billed except for medicare annual wellness visits, the price of a regular pharmacy consult will be lumped into the price of the visit with the geriatrician or NP.
  • Services are covered by most insurance plans.

Place:

  • Geriatric Clinic at Community Health Network 
  • Clinic will be open from 8-5 Monday-Friday
  • Pharmacist office will have adequate space to conduct consultations

Promotion:

  • We will have both internal and external promotion. Promotion among physicians in community health network to refer patients to our service, as well as external promotion so that patients or caregivers can identify a need for our service and enroll themselves.
  • Promote annual wellness visits to physicians in order to provide revenue for our service as well as decreasing physician workload

People:

  • Our clinic will consist of a Board Certified Geriatric Pharmacist, a Nurse Practioner, a geriatrician, and a social worker’s collaborative services

  • Professional and easy to use website
  • Flyer made for lobby/public areas in the clinic to promote service to patients

Process:

  • Documentation of visits will be made in the electronic medical record where the patient’s other providers in the Community Health Network can view their records.
  • Data will be collected on adverse events as a measure of patient safety, patient satisfaction scores and patient adherence.
