Conclusion

The establishment of this clinic will help insulin dependent patients take charge of their health and improve their health outcomes, all while embodying several of Community Health Network’s key values. Patients can expect better diabetic control through pharmacist intervention and patient education on U-500 insulin, insulin pumps, and/or continuous blood glucose monitoring. Additionally, reduced HgbA1c and hospitalizations will lead to decreased healthcare costs both to the patients and to Community Health Network. The endocrinology clinic and its providers will benefit from direct access to the pharmacist, who can assist with drug therapy recommendations and improving patient outcomes. The financial revenue provided by this clinic justifies its establishment, in addition to cost avoidance as a result of patients’ improved glycemic control. Based on existing physician buy-in, the initiation of this Intensive Insulin Clinic would fulfill Community Health Network’s need to better serve complex diabetic patients in the greater Indianapolis area.
