We propose to assist high-risk diabetic patients utilizing U-500 insulin, insulin pumps, and/or continuous blood glucose monitoring in controlling their diabetes while simultaneously providing cost avoidance for Community Health Network. This will be done through pharmacist interventions to achieve HgbA1C goals and prevent hospitalizations.
