    • Pharmacist run hepatitis C clinic with patients referred from providers seeing patients in the Eskenazi Gastroenterology/Hepatology Clinic beginning July 2017
    • Practice model that can positively influence hepatitis C outcomes, patient and provider satisfaction.
    • Pharmacists are uniquely qualified to educate patients on their disease state and medications as well as optimizing therapy clinically and economically.

  • Price
    • Cost avoidance/revenue on adjusted medications or improvement in care
    • Increase in amount of patients seen by existing HCV providers to increase clinic income

  • Place
    • Outpatient care center within Eskenazi Gastroenterology/Hepatology Clinic easily accessible to clinic patients
    • Individual patient rooms as well as team rooms with designated time for computer and office access

  • Promotion
    • Current part time pharmacist in Gastroenterology/Hepatology Clinic has generated positive promotion and physician champion to support hepatitis C clinic
    • Meeting with GI clinic staff to educate about pharmacist services and availability
    • Distribute handouts on Indiana Medicaid preferred medications to clinic staff (1)


 

  • People
    • Our staff: one pharmacist with internal medicine and/or ambulatory care experience, current administrative staff for clinic (i.e., prior authorization technician, scheduling), utilization of pharmacy students on rotations

  • Packaging
    • Patients will be referred to the clinic via an online referral form through Epic after being seen by a provider in the Gastroenterology/Hepatology Clinic
    • Material for patient education will be clear and concise.

  • Process
    • Documentation will be contained in patient’s EMR for all Eskenazi network professionals to access
    • Patient outcomes will be tracked based on vaccination status, time to treat, viral load, patient satisfaction/QOL, patient compliance, hospital admissions
