We propose to show how the addition of a pharmacist to the Gastroenterology/Hepatology Clinic at Eskenazi Health can improve patient outcomes and be an indispensable resource to the health care system. This service will aim to provide valuable patient education and assist in accessing cost effective hepatitis C medications with the ultimate goal of serving our vulnerable patient population.
